Cost is $13.00 for Birth, StillBirth Marriage and Devorce- $14.00 for Death. You must include the full names, if a woman her maiden name, the date and place, your relationship to the person or persons, and your signature. You also need a photocopy of a picture ID. There is no refund for a negative search.
